Do I have to input "chemotherapy" and/or "medication" when I am setting up a profile?

A.

No, you can add "chemotherapy" and/or "medication" to a profile at any time. Your profile name is the only item required to save a profile. However, if you do not add a chemotherapy or medication, you will not be able to note a chemotherapy or medication when creating diary entries. The Profile view is the only place to add chemotherapies and medications for use in diary entries.
