Coding for EMEND
Office-based physicians who dispense EMEND and other oral antiemetics to their patients for prechemotherapy and postchemotherapy prevention of nausea and vomiting may bill Medicare Part B for the covered drugs. CMS-1500 claim forms for oral drugs are filed to a DMERC or DME MAC rather than your local Part B carrier. You must have a DMERC or DME MAC supplier number to file claims for your region. Contact the National Supplier Clearinghouse at 866-238-9652 or visit the Web site for more information about obtaining a supplier number.
Providers must be enrolled as a Supplier of Durable Medical Equipment Prosthetic, Orthotics, and Supplies through the National Supplier Clearinghouse (NSC) to bill Part B drugs to the DME MAC for their region. For information about obtaining a supplier number, contact the NSC at 866-238-9652, or visit the Web site.

Please note: The National Uniform Claim Committee (NUCC) revised the CMS-1500 claim form. The form on this page represents the new version—CMS-1500 (08-05). The new version includes fields for the mandated National Provider Identifiers. The previous CMS-1500 (12-90) form did not have these fields. For further information, visit the CMS Web site or the NUCC Web site.

The efficacy of hormonal contraceptives may be reduced during coadministration with EMEND and for 28 days after the last dose of EMEND. Alternative or backup methods of contraception should be used during treatment with EMEND and for 1 month after the last dose of EMEND.
Coadministration of EMEND with warfarin may result in a clinically significant decrease in international normalized ratio (INR) of prothrombin time. In patients on chronic warfarin therapy, the INR should be closely monitored in the 2-week period, particularly at 7 to 10 days, following initiation of the 3-day regimen of EMEND with each chemotherapy cycle.
